ABSTRACT

A identificação precisa de nematódeos de interesse médico-veterinário em amostras de solo ou alimentos é uma ação complexa, devido à grande diversidade de nematódeos no ambiente. O objetivo do estudo foi realizar a caracterização morfológica e morfométrica de diferentes estágios larvários de ancilostomídeos, Strongyloides stercoralis, Strongyloides venezuelensis, Angiostrongylus cantonensis e Angiostrongylus costaricensis, e desenvolver um estudo de similaridade entre espécies do mesmo gênero, a fim de orientar o diagnóstico diferencial com os nematódeos ambientais. As larvas foram fixadas e diversos parâmetros corporais foram mensurados, para os quais foi realizada estatística descritiva e estudo de similaridade. Os resultados médios das larvas padrões foram compatíveis com os descritos para as espécies, com variabilidade em alguns parâmetros. S. stercoralis e S. venezuelensis demonstraram similaridade de 59% para L1 e 79% para L3, com maior diferenciação entre os grupos em relação à extremidade anterior e ao primórdio genital. No pool de larvas de ancilostomídeos, verificou-se uma grande variação nos parâmetros avaliados, especialmente quanto ao comprimento total do corpo. A. cantonensis e A. costaricensis apresentaram 48% de similaridade entre L1 e 76% para L3. Por meio da análise morfométrica e de similaridade verificou-se a possibilidade do aprimoramento do diagnóstico diferencial de nematódeos de origem ambiental. (AU)


Due to the enormous diversity of nematodes in the environment, identifying nematodes of medical and veterinary significance within soil or food samples can be challenging. To facilitate the differential diagnosis with environmental nematodes, this study aimed morphologically and morphometrically characterize various larval stages of hookworms, Strongyloides stercoralis, Strongyloides venezuelensis, Angiostrongylus cantonensi and Angiostrongylus costaricensis. Additionally, a similarity study was conducted among species within the same genus. The larvae were fixated and several body parameters were measured for the analysis, which included a comparison using descriptive statistics. Despite some parameter fluctuation, the average results for standard larvae were consistent with those of the respective species as a whole. The similarity between S. stercoralis and S. venezuelensis was 59% for L1 and 79% for L3, with more pronounced differences observed in the anterior extremity and the genital primordium. The examined parameters exhibited significant variation among the pool of hookworm larvae, particularly concerning the total length of the body. The similarities between A. cantonensis and A. costaricensis larvae ranged from 48% for L1 to 76% for L3. Analyses of morphometric and similarity data provided support for the potential improvement of differentiation between nematodes with environmental origins. This research contributes to enhancing the accuracy of identifying medically and veterinary relevant nematodes present in soil. (AU)

ABSTRACT

Objetivo: realizar análises parasitológicas em amostras de solo provenientes da represa da Bica situada no município de Catalão-GO. Métodos: a área do entorno da represa foi dividida em cinco parcelas, sendo realizadas coletas em dois pontos de cada parcela, superior e inferior. Foram realizadas quatro coletas em dias alternados, totalizando 40 amostras de, aproximadamente, 50g da camada superficial do solo, com profundidade máxima de cinco centímetros. As amostras foram processadas no Setor de Diagnóstico Parasitológico do Laboratório de Biologia Molecular da Universidade Federal de Catalão (UFCAT). Para as análises parasitológicas, foram utilizados cinco métodos encontrados na literatura, Rugai, Willis, Sedimentação espontânea, Ritchie e Ritchie modificado. Resultados: pelo método de Rugai, foi possível detectar larvas rabditoides do gênero Ancylostoma e Strongyloides. Utilizando o método de Ritchie modificado, foram encontrados ovos de Ascaris lumbricoidese Ancylostoma sp. Conclusão: a presença de formas evolutivas no solo da represa indica contaminação por dejetos animais e humanos, favorecendo o estabelecimento de ciclos biológicos das espécies encontradas e de outras diferentes zoonoses. Os dados obtidos demonstram a importância de sensibilizar a população de medidas de promoção da saúde, além de ações preventivas e programas de educação em saúde.


Objective: to conduct parasitological analyses in soil samples from the Bica reservoir located in the municipality of Catalão-GO. Methods: the area around the dam was divided into five plots, and samples were collected from two points in each plot, upper and lower. Four samples were collected on alternate days, totaling 40 samples of approximately 50g of topsoil, with a maximum depth of five centimeters. The samples were processed in the Parasitological Diagnostic Sector of the Molecular Biology Laboratory of the Federal University of Catalão (UFCAT). For the parasitological analyses, we used five methods found in the literature, Rugai, Willis, spontaneous sedimentation, Ritchie, and modified Ritchie. Results: by the Rugai method, it was possible to detect rhabditoid larvae of the genus Ancylostoma and Strongyloides. Using the modified Ritchie method, eggs of Ascaris lumbricoidesand Ancylostoma sp were found. Conclusion: the presence of evolutive forms in the soil of the reservoir indicates contamination by animal and human waste, favoring the establishment of biological cycles of the species found and of other different zoonoses. The data obtained demonstrated the importance of raising the population's awareness of health promotion measures besides preventive actions and health education programs.

Introducción: la miasis se define como la infestación de animales domésticos y salvajes, así como del hombre por estados larvarios de varias especies de dípteros que se alimentan de tejidos vivos o muertos o del alimento ingerido por el hospedero. Puede encontrarse en genitales, extremidades, cráneo, boca y párpados. Presentación de caso: se presenta el caso de un paciente, masculino de procedencia rural de 56 años de edad. Acude remitido de su área de salud por presentar varias ulceras a nivel del pene acompañado de aumento de volumen del mismo con secreciones fétidas, prurito y dolor. Se ingresa para estudio y tratamiento en sala. Se le diagnosticó miasis genital a nivel del glande. Tuvo una evolución satisfactoria en sala mediante curas locales y el tratamiento con antimicrobianos. Discusión: la miasis genital es poco común, probablemente por el aislamiento que ejerce la ropa al contacto con las moscas, sin embargo, la vestimenta corta predispone una mayor ovoposición en esta zona debido a una exposición más asequible, puede imitar lesiones inflamatorias o ulcerosas. De hecho, esta infestación parasitaria se ha asociado con determinadas enfermedades urogenitales de aspecto tumoral. Conclusiones: indiscutiblemente la inadecuada higiene de los genitales es un factor determinante en la aparición de la miasis.


Introduction: myiasis is defined as the infestation of domestic and wild animals, as well as man, by larval stages of various species of Diptera that feed on living or dead tissue or food ingested by the host. Genital myiasis is rare, of which there is little reference in the literature, specifically in the Caribbean. Case report: the case of a 56-year-old male patient of rural origin is presented. He was referred from his health area for presenting several ulcers on the penis accompanied by an increase in its volume with fetid secretions, itching and pain. He is admitted for study and treatment in the room. He was diagnosed with genital myiasis at the level of the glans. He had a satisfactory evolution in the room through local cures and treatment with antimicrobials. Discussion: genital myiasis is uncommon, probably due to the insulation exerted by clothing in contact with flies, however, short clothing predisposes a greater oviposition in this area due to more accessible exposure, it can mimic inflammatory or ulcerative lesions. In fact, this parasitic infestation has been associated with certain tumor-like urogenital diseases. Conclusions: unquestionably, inadequate genital hygiene is a determining factor in the appearance of myiasis.


Introdução: amiíase é definida como a infestação de animais domésticos e silvestres, assim como o homem, por fases larvais de várias espécies de Diptera que se alimentam de tecidos vivos ou mortos ou alimentos ingeridos pelo hospedeiro. Pode ser encontrado nos genitais, extremidades, crânio, boca e pálpebras. Apresentação do caso: apresenta-se o caso de um paciente do sexo masculino, 56 anos, de origem rural. Foi encaminhado de sua área de saúde por apresentar várias úlceras no pênis acompanhadas de aumento do seu volume com secreções fétidas, prurido e dor. Ele é admitido para estudo e tratamento na sala. Foi diagnosticado com miíase genital ao nível da glande. Teve evolução satisfatória na sala através de curas locais e tratamento com antimicrobianos. Discussão: amiíase genital é incomum, provavelmente devido ao isolamento exercido pelas roupas em contato com moscas, entretanto, roupas curtas predispõem uma maior oviposição nesta área devido à exposição mais acessível, podendo mimetizar lesões inflamatórias ou ulcerativas. De fato, essa infestação parasitária tem sido associada a certas doenças urogenitais semelhantes a tumores. Conclusões: inquestionavelmente, a higiene genital inadequada é um fator determinante no aparecimento da miíase.

ABSTRACT

Solos de praças públicas são comumente contaminados por helmintos devido ao fácil acesso de cães e gatos infectados. Esses animais ao defecarem podem liberar ovos desses parasitos e, em condições ambientais favoráveis, tornam-se ovos embrionados ou larvas infectantes. O objetivo deste trabalho foi investigar a existência de larvas de helmintos no solo de duas praças públicas do município de Caxias, Maranhão, Brasil, durante a estação chuvosa e seca na região. A pesquisa foi realizada em março de 2018, considerado período chuvoso, e em outubro do mesmo ano, período seco, sendo que foram coletadas trinta amostras de areia, quinze de cada praça, nos dois períodos do ano. O material foi coletado e levado para o Laboratório de Parasitologia do Departamento de Parasitologia e Microbiologia da Universidade Federal do Piauí para análise. Na estação chuvosa, das quinze amostras analisadas na praça A, cinco foram positivas para larvas de ancilostomídeos e das quinze na praça B, três estavam contaminadas com os mesmos helmintos. No período seco, na praça A havia apenas uma amostra com essas larvas e na praça B não foram encontrados parasitos. Os resultados revelaram a presença de larvas de helmintos de caráter zoonótico no solo de praças públicas de Caxias, Maranhão, principalmente no período chuvoso, servindo de alerta à população local.(AU)


Soil in public squares is commonly contaminated by helminths due to the easy access of infected dogs and cats. These animals, when defecating, can release helminth eggs and, under favorable environmental conditions, those eggs can become embryonated or infective larvae. The purpose of this work was to investigate the existence of helminth larvae in the soil of two public squares in the city of Caxias, in the state of Maranhão, Brazil, during the rainy and dry seasons in the region. The study was carried out in March 2018, which is considered the rainy season, and in October of the same year, the dry season. A total of thirty sand samples were collected, fifteen from each square, in both periods of the year. The material was collected and taken to the Parasitology Laboratory of the Department of Parasitology and Microbiology of the Federal University of Piauí for analysis. In the rainy season, from the fifteen samples analyzed in square A, five were positive for hookworm larvae; and from the fifteen samples collected from square B, three were contaminated with the same helminths. During the dry period, only one sample from square A presented these larvae while no parasites were found in square B. The results revealed the presence of zoonotic helminth larvae in the soil of public squares in Caxias, Maranhão, mainly in the rainy season, which can be used as a warning sign to the local population.(AU)


Los suelos de las plazas públicas son comúnmente contaminados por helmintos debido al fácil acceso de perros y gatos infectados. Esos animales, al defecar, pueden liberar huevos de esos parásitos y, en condiciones ambientales favorables, convertirse en huevos embrionados o larvas infectantes. El objetivo de este trabajo fue investigar la existencia de larvas de helmintos en el suelo de dos plazas públicas de la ciudad de Caxias, Maranhão, Brasil, durante la estación lluviosa y seca de la región. La investigación se realizó en marzo de 2018, considerada época de lluvias, y en octubre del mismo año, época seca, y se recolectaron treinta muestras de arena, quince de cada plaza, en ambos períodos del año. El material fue recolectado y llevado al Laboratorio de Parasitología del Departamento de Parasitología y Microbiología de la Universidad Federal de Piauí para su análisis. En época de lluvias, de las quince muestras analizadas en la plaza A, cinco resultaron positivas a larvas de anquilostomiasis y de las quince de la plaza B, tres estaban contaminadas con los mismos helmintos. En el período poco lluvioso, en la plaza A solo hubo una muestra con esas larvas y en la plaza B no se encontraron parásitos. Los resultados revelaron la presencia de larvas de helmintos zoonóticos en el suelo de las plazas públicas de Caxias, Maranhão, principalmente en la época de lluvias, sirviendo de alerta a la población local.(AU)

Objetivo:Analisar as evidências disponíveis na literatura acerca da utilização da larvoterapia no tratamento de feridas de difícil cicatrização. Método: Trata-se de revisão integrativa, tendo como questão norteadora: quais são as evidências disponíveis na literatura acerca da utilização da larvoterapia no tratamento de feridas de difícil cicatrização? Elegeram-se como critérios de inclusão: artigos originais, de revisão de literatura e de relatos de experiência, recorte temporal de 2016 a 2021, disponíveis online na íntegra, em português, inglês e espanhol e que respondessem à pergunta de pesquisa. Os critérios de exclusão foram: artigos duplicados, cartas ao editor, dissertações, teses, monografias e pesquisas com animais. Buscas ocorreram na Biblioteca Virtual em Saúde e Biblioteca Nacional de Medicina dos Estados Unidos entre agosto e setembro de 2021. Resultados: Selecionaram-se 10 artigos. As principais vantagens foram: desbridamento seletivo, ação bactericida ou bacteriostática e promoção de tecido de granulação e de fatores cicatrizantes. Já as principais desvantagens abrangeram: dor, desconforto, preconceito e fragilidades com o mercado de produção larval. Conclusão: As vantagens encontradas sobrepõem as desvantagens evidenciadas. Portanto, a larvoterapia é favorável para tratar feridas de difícil cicatrização.


Objective:To analyze the evidence available in the literature about the use of larvotherapy in the treatment of difficult-to-heal wounds. Method: This is an integrative review, having as a guiding question: what evidence is available in the literature about the use of larvotherapy in the treatment of wounds that are difficult to heal? The inclusion criteria were: original articles, literature review and experience reports, from 2016 to 2021, available online in full, in Portuguese, English and Spanish and that answered the research question. Exclusion criteria were: duplicate articles, letters to the editor, dissertations, theses, monographies and research with animals. Searches took place at Virtual Health Library and United Station National Library of Medicine between August and September 2021. Results: Ten articles were selected. The main advantages were: selective debridement, bactericidal or bacteriostatic action, and promotion of granulation tissue and healing factors. The main disadvantages were: pain, discomfort, prejudice, and weaknesses with the larval production market. Conclusion: The advantages found outweigh the disadvantages evidenced. Therefore, larvotherapy is favorable to treat wounds that are difficult to heal.


Objetivo:Analizar la evidencia disponible en la literatura sobre el uso de la larvoterapia en el tratamiento de heridas de difícil cicatrización. Método: Se trata de una revisión integradora, teniendo como pregunta orientadora: ¿cuál es la evidencia disponible en la literatura sobre el uso de la larvoterapia en el tratamiento de heridas de difícil cicatrización? Los criterios de inclusión fueron: artículos originales, revisión de la literatura y relatos de experiencia, período de tiempo de 2016 a 2021, disponibles en línea en su totalidad, en portugués, inglés y español y que respondieron a la pregunta de investigación. Los criterios de exclusión fueron: artículos duplicados, cartas al editor, disertaciones, tesis, monografías e investigaciones con animales. Las búsquedas se realizaron en la Biblioteca Virtual de Salud y la Biblioteca Nacional de Medicina de los Estados Unidos entre agosto y septiembre de 2021. Resultados: se seleccionaron 10 artículos. Las principales ventajas fueron: desbridamiento selectivo, acción bactericida o bacteriostática y promoción del tejido de granulación y factores de cicatrización. Las principales desventajas incluyeron: dolor, incomodidad, prejuicio y debilidades con el mercado de producción de larvas. Conclusión: Las ventajas encontradas superan las desventajas destacadas. Por lo tanto, la larvoterapia es favorable para tratar heridas de difícil cicatrización.

Introducción. La creciente resistencia bacteriana a los antibióticos representa una amenaza mundial de salud pública. Las excreciones y secreciones larvarias derivadas de moscas necrófagas de la familia Calliphoridae podrían configurar una fuente promisoria para contrarrestar sus efectos. Objetivo. Comparar la actividad antimicrobiana de las excreciones y secreciones larvarias nativas, y de las mayores y menores de 10 kDa de Calliphora vicina y Sarconesiopsis magellanica (Diptera: Calliphoridae). Materiales y métodos. El bioensayo se hizo a partir de la técnica de turbidimetría y en el caso de las excreciones y secreciones menores de 10 kDa se determinó la concentración inhibitoria mínima (CIM). Resultados. Las excreciones y secreciones nativas y las menores de 10 kDa de C. vicina y S. magellanica, evidenciaron una potente actividad antibacteriana contra tres cepas de Staphylococcus aureus y cuatro bacterias Gram negativas, siendo las menores de 10 kDa más efectivas que las nativas en las dos especies de moscas evaluadas. Además, las menores de 10 kDa presentaron la misma efectividad, aunque en las pruebas de CIM se observó que las de S. magellanica fueron más potentes en todas las bacterias evaluadas, excepto contra la cepa de S. aureus ATCC 25923. Las mayores de 10 kDa no inhibieron el crecimiento bacteriano. Conclusión. Los resultados validaron, en general, que estas sustancias son fuente importante para el aislamiento y la caracterización de agentes antimicrobianos.


Introduction: The growing resistance to antibiotics worldwide represents a global threat to public health. The larval excretions and secretions derived from necrophagous flies from the Calliphoridae family could represent a promising source for counteracting their effects. Objective: To compare the antimicrobial activity of Calliphora vicina and Sarconesiopsis magellanica (Diptera: Calliphoridae) native excretions and secretions and those weighing more than 10 kDa and less. Materials and methods: We used the turbidimetry technique for the bioassay; we determined the minimum inhibitory concentration (MIC) for excretions and secretions weighing less than 10 kDa. Results: Calliphora vicina and S. magellanica native excretions and secretions and those weighing less than 10 kDa exhibited potent antibacterial activity against three Staphylococcus aureus strains and four Gram-negative bacteria; those weighing less than 10 kDa were more effective than the native ones in the two species of flies evaluated here. Furthermore, excretions and secretions weighing less than 10 kDa had the same effectiveness, except in the MIC trials where S. magellanica excretions and secretions weighing less than 10 kDa were more potent against all the bacteria evaluated, except for S. aureus ATCC 25923. Excretions and secretions weighing more than 10 kDa did not inhibit bacterial growth. Conclusions: These results potentially validate these substances as an important source for isolating and characterizing antimicrobial agents.

Uma ampla variedade de preenchimentos dérmicos está agora disponível para uso na indústria da beleza. Apresentamos o caso de uma mulher de 33 anos com complicações após injeção de ácido hialurônico por um não médico. Duas semanas após a injeção, dor nos seios da face bilateral, febre e sensação de queimação ocorreram na área da bochecha. Ela foi submetida a um diagnóstico detalhado revelando larva de Attagenus nos seios paranasais. Apesar do tratamento, os sintomas continuaram presentes, sendo administrada hialuronidase na região infraorbital com remissão completa


A wide range of dermal fillers is now available for use in the beauty industry. We present a case of a 33-year-old woman with complications after hyaluronic acid injection by a non-medical practitioner. Two weeks after the injection, bilateral sinus pain, fever, and burning sensation occurred in the cheek area. She underwent a detailed diagnosis revealing Attagenus larva in the paranasal sinus. Despite treatment, the symptoms remained. Thus, hyaluronidase was administered in the infraorbital area, obtaining a full remission.

Abstract Millions of people worldwide, and especially schoolchildren, may be infected by geohelminths due to their exposure to a contaminated environment. The aim of this study was to evaluate soil contamination by Ancylostoma spp. and Toxocara spp. eggs in recreation areas at elementary schools in Pelotas, state of Rio Grande do Sul, Brazil. Sand samples were collected from 22 schools and were processed using the centrifugal flotation method. Helminth eggs with zoonotic potential were found in 12 out of the 22 schools (54.5%). Contamination by Ancylostoma spp. and Toxocara spp. was observed in 36.4% (8/22) and 27.3% (6/22) of the soil samples collected at these schools, respectively. These findings of eggs show that the school communities are exposed to risks of zoonotic transmission.


Resumo Milhões de pessoas podem ser acometidas por geohelmintos, especialmente crianças em idade escolar, devido a sua maior exposição a ambientes contaminados. O objetivo deste estudo foi avaliar a contaminação do solo por ovos de Ancylostoma spp. e Toxocara spp. em áreas de recreação de escolas de ensino fundamental da cidade de Pelotas, Rio Grande do Sul, Brasil. Foram colhidas amostras de areia de 22 escolas e processadas pelo método de centrífugo-flutuação. Em 54,5% (12/22) das escolas houve registro da presença de ovos de helmintos com potencial zoonótico. A contaminação por Ancylostoma spp. e Toxocara spp. foi observada em 36,4% (8/22) e 27,3% (6/22) das amostras de solo das escolas, respectivamente. Existe a presença de ovos de Ancylostoma spp. e Toxocara spp., havendo risco de transmissão de zoonoses à comunidade escolar.

@#Abstract: Objective To compare the efficiency of lung-microscopy and tissue homogenate in the detection of Angiostrongylus cantonensis larvae from Pomacea canaliculate, with the amin of finding a simple and rapid method suitable for different scenarios. Methods Pomacea canaliculata was caught and collected from ponds, ditches, rivers and other environments in the confirmed epidemic areas reported by predecessors. After each snail was weighed one by one, and dissected into two parts: lung sac and muscle. Firstly, each snail lung sac was check for nodules with lung-microscopy, and the nodules were picked out with anatomical needle and press them separately. Then, the worm was found and identified for worm species under microscope, and the lung sac and snail meat then was rechecked with tissue homogenate. Results A total of 330 snails were detected, with 19.1% (63/330) snails with Angiostrongylus cantonensis stage Ⅲ larvae were detected by tissue homogenate and 15.8% (52/330) snails with nodules were detected by lung-microscopy. Among them, 36 snails with nodules and larvae were detected by the lung-microscopy, and all of them were positive by the tissue homogenate, with a coincidence rate of 100% (36/36); 16 snails with nodules but no larvae, among which 6 snails were positive and 10 snails were negative by the tissue homogenate, The false detection rate was 19.2% (10/52). 278 snails with no nodules, but 21 of them were detected by the tissue homogenate, the missing rate was 7.6% (21/278). There was no significant difference between tissue homogenate and lung-microscopy (taking the positive determination of nodules as the standard) (χ2=1.27, P=0.26, P>0.05). There was significant difference between tissue homogenate and lung-microscopy (taking the detection of larvae as the standard)(χ2=8.66, P = 0.003, P<0.01). There was no significant difference between the two methods and tissue homogenate in the detection rate of large snails ( ≥25 g, χ2=0.08,P=0.777; χ2=2.58, P=0.108), but there was significant difference between the two methods and tissue homogenate in the detection rate of small snail (≤10 g, χ2=5.63, P=0.02). Conclusions Compared with the tissue homogenate, lung-microscopy is simple in the detection of large snails, requires less instruments and equipment, and its detection speed is faster. It is suitable for the field investigation of the natural focus of Angiostrongylus cantonensis. The tissue homogenate has high sensitivity, can directly display the insect state and activity, has strong insect vitality and high detection rate, it's more suitable for food safety risk monitoring.

Objective To investigate the resistance development of Aedes albopictus to commonly used insecticides in Yangpu District of Shanghai, China, and to provide scientific bases for the control of A. albopictus and the emergency treatment of dengue fever. Methods The larva immersion method recommended by WHO was used to assay the median lethal concentration (LC50) of A. albopictus larva. SPSS 25.0 was used to calculate the toxicity baselines of insecticides. Results A. albopictus kept a medium resistance to beta-cypermethrin, and the resistance ratios were 14.44, 10.00, 17.78, respectively, in 2015, 2017, 2019. The mosquitos kept a high resistance to permethrin, and the resistance ratios were 54.00, 140.00, 52.00 respectively, in 2015, 2017, 2019. Besides, A. albopictus also kept a medium resistance to propoxur, and the resistance ratios were 15.34, 13.06, 10.83 respectively, in 2015, 2017, 2019. To deltamethrin, A. albopictus showed high resistance in 2015 and medium resistance in 2017, the resistance ratios were 40.00 and 35.00. To temephos, A. albopictus showed low resistance in 2015, with the resistance ratio of 9.38. Conclusion A. albopictus has developed high resistance to permethrin in Yangpu District of Shanghai, and cautious and reduced use of permethrin is suggested. It is necessary to implement and adopt the strategy of comprehensive management, in order to establish the long-term mechanism for mosquito control and prevention.

INTRODUCCIÓN: La toxocariasis es una enfermedad parasitaria ampliamente distribuida. Las formas clínicas descritas son la asintomática (TA), larva migrans visceral (LMV), larva migrans ocular (LMO) y encubierta (TE). OBJETIVOS: Describir las características clínicas, de laboratorio, evolución y tratamiento de los casos y comparar las diversas formas clínicas de presentación. PACIENTES Y MÉTODOS: Análisis retrospectivo de todos los pacientes pediátricos con diagnóstico de toxocariasis atendidos en el Servicio de Infectología del Hospital General de Niños Pedro de Elizalde entre 2012 y 2019. RESULTADOS: Se incluyeron 85 pacientes. El 63,5 % fueron varones y la mediana de edad fue de 60 meses. Cuarenta y nueve pacientes presentaron TA, 14 LMV, 15 LMO y 7 TE. Los pacientes con LMV presentaron menor edad y recuento de eosinófilos más altos. Todos los casos de LMO presentaron mal pronóstico visual. Se indicó tratamiento con albendazol en todos los casos de LMV, en los casos activos de LMO, en 4 TE y en 3 TA. CONCLUSIÓN: Este estudio representa uno de los más grandes llevados a cabo en el país. Todas las formas clínicas excepto las LMO tuvieron buena evolución. Resulta fundamental enfatizar en la prevención y en el diagnóstico precoz de la enfermedad para instaurar de manera oportuna el tratamiento y evitar secuelas.


BACKGROUND: Toxocariasis is a widely spread parasitic disease. The most frequent clinical form is asymptomatic (AT) although it may present with visceral larva migrans (VLM), ocular larva migrans (OLM) or covert (TE) involvement. Aims: To describe the clinical presentation, laboratory, evolution and treatment characteristics of the cases and to compare the various clinical forms of presentation. PATIENTS AND METHODS: Retrospective analysis of all children diagnosed with toxocariasis attended at the Infectology Service of the Pedro de Elizalde Children's General Hospital between 2012-2019. RESULTS: We included 85 patients. 63.5% were males and the median age was 60 months. 49 patients presented AT, 14 VLM, 15 OLM and 7 TE. Children with LMV had lower age and higher eosinophil count. All the cases of OLM evolved with a poor visual prognosis. Treatment with albendazole was indicated in all cases of LMV, in active cases of LMO, in 4 TE and in 3 AT. CONCLUSION: This study represents one of the largest conducted in our country. The ocular forms had bad prognosis, while the visceral and covert forms had good evolution. It is essential to emphasize the prevention and early diagnosis of the disease in order to establish timely treatment and avoid sequelae.

RESUMEN Toxocara canis y catis son helmintos que parasitan el intestino del perro o del gato, sus huéspedes definitivos. Las hembras eliminan huevos infectantes al medio ambiente, que, ingeridos accidentalmente por el ser humano, sólo desarrollan el estado larvario en etapa embrionaria y causan la toxocariasis. Esta es una zoonosis extendida por todo el mundo, especialmente en aquellas áreas donde prevalecen condiciones deficientes de hábitat e higiene. El cuadro clínico puede variar desde formas asintomáticas a la larva migrans visceral (LMV) o larva migrans ocular (LMO), debidas a la diseminación de larvas embrionadas en el hígado, pulmones y en menor medida el SNC. La LMV se caracteriza clínicamente por la presencia de hepatomegalia, infiltración pulmonar con tos, eosinofilia de variada intensidad, hiper Ig E y anticuerpos Ig G anti-toxocara. Por no ser la fiebre un síntoma clínico destacado, se presenta el siguiente caso clínico.

Females shed infective eggs into the environment, which, when accidentally ingested by humans, only develop the embryonic larval stage and cause toxocariasis. This is a zoonosis that is widespread throughout the world, especially in those areas where poor habitat and hygienic conditions prevail. The clinical picture can vary from asymptomatic forms to visceral larva migrans (VLM) or ocular larva migrans (OLM), due to the dissemination of embryonic larvae in the liver, lungs and to a lesser extent the CNS. VLM is clinically characterized by the presence ofhepatomegaly, pulmonary infiltration with cough, eosinophilia of varying intensity, hyper Ig E and Ig G anti-toxocara antibodies. Since fever is not a prominent clinical symptom, the following clinical case is presented.

Abstract Background: Myiasis is an emerging disease caused by tissue invasion of dipteran larvae. In Uruguay, Cochliomyia hominivorax and Dermatobia hominis are the most frequent species. This study aimed to describe the epidemiological and clinical characteristics and the follow-up of children < 15 years hospitalized for myiasis in a reference center in Uruguay between 2010 and 2019. Methods: We conducted a descriptive and retrospective study by reviewing medical records. We analyzed the following variables: age, sex, comorbidities, origin, the month at admission, clinical manifestations, other parasitoses, treatments, complications, and larva species identified. Results: We found 63 hospitalized children: median age of 7 years (1 month-14 years), 68% of females. We detected risk comorbidities for myiasis (33%), of which chronic malnutrition was the most frequent (n = 6); 84% were from the south of the country; 76% were hospitalized during the summer. Superficial and multiple cutaneous involvements were found in 86%: of the scalp 50, furunculoid type 51, secondary to C. hominivorax 98.4%, and to D. hominis in 1.6%. As treatments, larval extraction was detected in all of them, surgical in 22%. Asphyctic products for parasites were applied in 94%, ether in 49. Antimicrobials were prescribed in 95%; cephradine and ivermectin were the most frequent. About 51% presented infectious complications: impetigo was found in 29, cellulitis in 2, and abscess in 1. Conclusions: Myiasis predominantly affected healthy schoolchildren during the summer months and was frequently associated with ectoparasites. Superficial infestation of the furuncle-like scalp by C. hominivorax was the most frequent form. Infectious complications motivated hospitalizations.


Resumen Introducción: La miasis es una enfermedad emergente producida por la invasión tisular por larvas de dípteros. En Uruguay, Cochliomyia hominivorax y Dermatobia hominis son las especies más frecuentes. El objetivo de este estudio fue describir las características epidemiológicas y clínicas, y el seguimiento, de niños menores de 15 años hospitalizados por miasis en un centro de referencia del Uruguay entre 2010 y 2019. Métodos: Se llevó a cabo un estudio descriptivo y retrospectivo mediante la revisión de las historias clínicas. Se analizaron las siguientes variables: edad, sexo, comorbilidad, procedencia, mes al ingreso, cuadro clínico, otras parasitosis, tratamientos, complicaciones y especie de larva identificada. Resultados: Se encontraron 63 casos de niños hospitalizados, con una mediana de edad de 7 años (rango: 1 mes a 14 años) y el 68% de sexo femenino. Se identificó comorbilidad de riesgo para miasis (33%), siendo la desnutrición crónica la más frecuente (n = 6). El 84% eran procedentes del sur del país y el 76% fueron hospitalizados en los meses estivales. Se encontró afectación cutánea superficial y múltiple en el 86%: de cuero cabelludo en 50, de tipo forunculoide en 51, secundaria a C. hominivorax en el 98.4% y a D. hominis en el 1.6%. Se realizó extracción larvaria en todos los casos, que fue quirúrgica en el 22%. Se aplicaron productos asfícticos para parásitos en el 94%, con éter en 49 niños. Se prescribieron antimicrobianos en el 95%; los más usados fueron cefradina e ivermectina. El 51% presentaron complicaciones infecciosas: 29 impétigo, 2 celulitis y 1 absceso. Conclusiones: La miasis afectó predominantemente a escolares sanos y durante los meses estivales, y se encontró frecuentemente asociada con ectoparasitosis. La infestación superficial del cuero cabelludo de tipo forunculoide por C. hominivorax fue la forma más frecuente. Las complicaciones infecciosas motivaron las hospitalizaciones.

Resumen Introducción: Hamadryas feronia feronia, se mimetiza sobre las cortezas de los árboles y suele emitir sonidos al volar que llaman la atención; posee un buen potencial para los bionegocios (exportación, elaboración de artesanías y centros turísticos de crianza). Sin embargo, aún se desconocen sus aspectos biológicos que obstaculizan su crianza en cautiverio. Objetivos: Determinar los aspectos biológicos de alimentación, comportamiento de oviposición, ciclo de vida y los enemigos naturales de Hamadryas feronia feronia L. en San Rafael-Indiana, Loreto, Perú. Métodos: Los muestreos fueron realizados desde enero 2018 a diciembre 2019 en la comunidad de San Rafael, río Amazonas. Los adultos fueron observados durante el día, se registraron sus plantas alimenticias, su comportamiento de oviposición, su ciclo biológico y sus enemigos naturales. El ciclo de vida fue evaluado en el laboratorio, utilizando 20 huevos recientemente depositados en las hojas de su planta hospedera. Resultados: Las larvas de H. feronia feronia se alimentan de las hojas de Dalechampia juruana y los adultos se alimentan de la savia de la corteza de los árboles de Cedrela odorata, Spondias mombin, Uncaria guianensis y de los frutos fermentados de Syzygium malaccense y Pouteria caimito. Los adultos vuelan en días soleados, los machos emiten un fuerte sonido al volar. Las hembras previo a la oviposición revolotean de forma irregular alrededor de su planta hospedera entre las 8.00 y las 14.00 h y depositan sus huevos en el haz y envés de las hojas de forma aislada con mayor frecuencia en el envés (N= 85). La duración del ciclo, desde huevo hasta adulto fue de 28.24 días. El periodo promedio del huevo fue 3.75 ± 0.40 días. La larva pasa por cinco estadíos larvales: el primero duró 3.21 ± 1.03 días, el segundo 2.78 ± 0.73 días, el tercero 2.67 ± 0.77 días, el cuarto 3.22 ± 0.81 días, y el quinto 4.61 ± 0.70 días. El periodo de la prepupa duró 1.33 ± 0.49 días y el de pupa 6.67 ± 0.80 días; los adultos nacieron entre las 10:00 y 11:00 h. Los machos adultos viven en promedio 31.80 ± 3.29 días, la hembra 42.00 ± 2.14 días y sus huevos son parasitados por un microhimenóptero (Scelionidae). Conclusiones: Este estudio permitió conocer los aspectos biológicos de H. feronia feronia identificando sus plantas alimenticias tanto de las larvas como de los adultos, su comportamiento de oviposición, así mismo se ha determinado que tiene un ciclo biológico relativamente corto con un periodo menor de un mes y sus huevos son consumidos por un pequeño Himenóptero que puede obstaculizar su producción. Este trabajo brinda información necesaria para desarrollar la crianza de H. feronia feronia, orientado a su conservación, la educación ambiental y los bionegocios (turismo y artesanía) en la Amazonia peruana.


Abstract Introduction: Hamadryas feronia feronia, which mimics the bark of trees and often makes attention-grabbing sounds when flying, has good potential for bio-business (export, handicrafts and tourist breeding centers). However, its biological aspects are still unknown, which hinder its captive breeding. Objectives: To determine the biological aspects of feeding, oviposition behavior, life cycle and natural enemies of Hamadryas feronia feronia L. in San Rafael-Indiana, Loreto, Peru. Methods: Sampling was conducted from January 2018 to December 2019 in the community of San Rafael, Amazon River. Adults were observed during the day, their food plants, oviposition behavior, biological cycle and natural enemies were recorded. The life cycle was evaluated in the laboratory, using 20 eggs recently deposited on the leaves of their host plant. Results: The larvae of H. feronia feronia feed on the leaves of Dalechampia juruana and the adults feed on the bark sap of Cedrela odorata, Spondias mombin, Uncaria guianensis and the fermented fruits of Syzygium malaccense and Pouteria caimito. Adults fly on sunny days, males emit a loud sound when flying. Females prior to oviposition flit irregularly around their host plant between 8.00 and 14.00 h and deposit their eggs on the upper and underside of leaves in isolation, most frequently on the underside. The duration of the cycle from egg to adult was 28.24 days. The average egg period was 3.75 ± 0.40 days. The larvae passed through five larval instars: the first instar 3.21 ± 1.03 days, the second 2.78 ± 0.73 days, the third 2.67 ± 0.77 days, the fourth 3.22 ± 0.81 days, and the fifth 4.61 ± 0.70 days. The prepupal period lasted 1.33 ± 0.49 days and the pupal period 6.67 ± 0.80 days; the adults hatched between 10:00 to 11:00 h. Adult males lived on average 31.80 ± 3.29 days, the female 42.00 ± 2.14 days and their eggs were parasitized by a microhymenopteran (Scelionidae). Conclusions: This study allowed to know the biological aspects of H. feronia feronia identifying its food plants of both larvae and adults, its oviposition behavior, as well as it has been determined that it has a relatively short biological cycle with a period of less than one month and its eggs are consumed by a small Hymenoptera that can hinder its production. This work provides necessary information to develop the breeding of H. feronia feronia, oriented to its conservation, environmental education and biotrade (tourism and handicrafts) in the Peruvian Amazon.

RESUMEN La toxocariasis humana es una importante zoonosis parasitaria causada por formas larvarias del toxocara, un parásito nematodo de perros y gatos. La migración de la larva por los diferentes tejidos blandos en el ser humano genera una serie de entidades clínicas, tales como: toxocariasis encubierta, el síndrome de larva migrans visceral, la toxocariasis ocular y la neurotoxocariasis. Se presenta un caso clínico de un paciente de seis años de edad, hospitalizado a consecuencia de un síndrome febril agudo asociado a dolor abdominal; cuyo examen físico, reveló signos meníngeos, fotofobia, alteraciones en el fondo de ojo y eosinofilia moderada. Teniendo en cuenta la epidemiología, el cuadro clínico y los resultados de estudios serológicos, se concluyó el diagnóstico de larva migrans ocular, para lo cual se utilizó tratamiento específico con una evolución satisfactoria. Esta entidad nosológica requiere de una adecuada valoración socio-epidemiológica de los pacientes, así como de un minucioso examen físico que, unido a las pruebas específicas permitirán el diagnóstico precoz y el tratamiento oportuno.


ABSTRACT Human toxocariasis is an important parasitic zoonosis caused by larval forms of toxocara, a nematode parasite of dogs and cats. The migration of the larva through the different soft tissues in humans generates a series of clinical entities, such as: covert toxocariasis, visceral larva migrans syndrome, ocular toxocariasis and neurotoxocariasis. The clinical case of a six-year-old patient is presented, hospitalized as a result of an acute febrile syndrome associated with abdominal pain; whose physical examination revealed meningeal signs, photophobia, fundus alterations and moderate eosinophilia. Taking into account the epidemiology, the clinical picture and the results of serological studies, the diagnosis of ocular larva migrans was concluded, for which specific treatment was used with a satisfactory evolution. This nosological entity requires an adequate socio-epidemiological assessment of the patients, as well as a thorough physical examination that, together with the specific tests, will allow early diagnosis and timely treatment.

La dermatosis denominada larva migrans cutánea: (LMC) es una infección cutánea secundaria a infestación parasitaria por la migración de larvas de anquilostomas animales a la epidermis humana, frecuentemente por contacto directo con suelos contaminados, especialmente en zonas tropicales. Se caracteriza por la aparición de lesiones induradas, eritematosas con patrón irregular o serpiginoso acompañado de prurito. Describimos un caso de LMC en una niña, adquirido durante unas vacaciones en Colombia y tratado inicialmente como celulitis con antibióticos (AU)


Cutaneous larva migrans (CLM) is an infection secondary to parasitic infestation due to the migration of animal hookworm larvae into the human skin, frequently by direct contact with contaminated grounds, especially in the tropics. Clinically, it is characterized by the appearance of indurated, erythematous lesions with irregular or "creeping eruption" pattern and pruritus. This article describes a case of CLM infection in a pediatric patient, it was acquired during the holidays in Colombia, diagnosed as cellulitis and treated accordingly with antibiotics without success (AU)

ABSTRACT Objective: To report the case of an infant with infrequent cranial osteomyelitis as a complication of furuncular myiasis. Case description: The patient was a 4-month-old male who presented to the emergency department with a nodular skull lesion with edema, tenderness, pain, and purulent drainage, as well as progress of the ulcerated lesion and evidence of larvae inside. Antibiotic treatment was initiated, and the patient was taken to the operating room to remove the larvae, but he had no symptomatic improvement. A skull radiograph was taken to visualize the osteolytic lesion, and a 3D computed tomography scan showed osteomyelitis of the external parietal surface. Antibiotic management readjustment continued for a total of six weeks, and a skin flap was used with clinical improvement. Comments: Myiasis is defined as the infestation of vertebrates with fly larvae. In mammals, larvae can feed on host tissue and cause a wide range of infestations depending on their location in the body. The cranial osteomyelitis as a complication of myiasis described in this report seems to be an exceptional case.


RESUMO Objetivo: Relatar um caso de criança com osteomielite craniana infrequente como complicação da miíase furuncular. Descrição do caso: Paciente do sexo masculino, com quatro meses de idade, que se apresentou no pronto-socorro com lesão nodular no crânio com edema, sensibilidade, dor e drenagem purulenta, com evolução da lesão ulcerada e evidência de larva no interior. O tratamento com antibióticos foi iniciado e o paciente foi levado à sala de cirurgia para remover as larvas, mas não houve melhora. Uma radiografia do crânio foi realizada para visualizar a lesão osteolítica e uma tomografia computadorizada em 3D mostrou osteomielite da superfície parietal externa. O reajuste do tratamento com antibióticos foi mantido por um total de seis semanas e um retalho cutâneo foi realizado com melhora clínica. Comentários: Miíase é definida como a infestação de vertebrados com larvas de moscas. Nos mamíferos, as larvas podem se alimentar do tecido hospedeiro e causar uma ampla variedade de infestações, dependendo da sua localização no corpo. A osteomielite como complicação da miíase, apresentada nesse caso, parece ser uma forma não usual de complicação dessa doença.

Objective:To observe the therapeutic effect of medical maggot on diabetic foot, and analyze its possible mechanism.Methods:Sixty diabetic foot ulcer patients with Wagner grade 1 or 2 from July 2018 to September 2019 in Second Affiliated Hospital of Dalian Medical University were selected. The patients were divided into control group and experimental group by random digits table methods with 30 cases in each group. The control group was treated with traditional method, and the experimental group was treated with medical maggots. In experimental group, the wound surface Notch 1 mRNA expression before treatment and at the 4th day of treatment was detected by polymerase chain reaction method. The recovery rate 21 d after treatment was compared between 2 groups.Results:The recovery rate in experimental group was significantly higher than that in control group: 100.00% (30/30) vs. 76.67% (23/30), and there was statistical difference ( χ2 = 7.925, P<0.01). In experimental group, there was no statistical difference in the wound surface Notch 1 mRNA expression between before treatment and the 4th day of treatment (0.96 ± 0.08 vs. 0.93 ± 0.08; t = 1.545, P>0.05). Conclusions:Medical maggot has an obvious effect on Wagner grade 1 or 2 diabetic foot ulcers, but its mechanism maybe unrelated to Notch1 signaling pathway.

ABSTRACT

Resumen: La miasis se produce por la infestación con larvas de especies de moscas que afectan los tejidos u órganos de los vertebrados, incluido el ser humano. Puede clasificarse por caracterización entomológica o según el tropismo de las larvas en los tejidos. La miasis intestinal es poco frecuente y de difícil diagnóstico dada su sintomatología inespecífica, por lo que la única forma de confirmar el caso es mediante la identificación de las larvas expulsadas. Se presenta el primer caso reportado en el departamento de Nariño (Colombia) de miasis intestinal en un niño de seis años residente en la zona urbana del municipio de Policarpa, proveniente de una familia de nivel socioeconómico bajo, sin acceso a un adecuado tratamiento y sin disposición de aguas residuales y con insuficientes condiciones de saneamiento básico. El caso clínico se asoció con diarrea crónica, dolor abdominal y prurito anal, con la posterior expulsión de una larva cuyas características morfológicas correspondían a las de la mosca Eristalis tenax.


Abstract: Myiasis is a pathology caused by the infestation of fly larvae species which affects vertebrates' tissues or organs including humans. It can be classified entomologically or according to tissue tropism. Intestinal myiasis is rare and difficult to diagnose given its non-specific symptoms; the only way to confirm a case is by identifying the expelled larvae. This is the first case of intestinal myiasis reported in the department of Nariño (Colombia) in a 6-year-old child from a low-income family in the urban area of the municipality of Policarpa where there is no adequate treatment and no disposition of wastewater and basic sanitation conditions are insufficient. The clinical case was related to chronic diarrhea, abdominal pain, and anal pruritus with the subsequent expulsion of a larva identified by its morphological characteristics as Eristalis tenax fly larva.
